Katrina Hill

Katrina is a Lead Social Prescribing Link Worker managing a growing team of Social Prescribers in Torbay. Social Prescribing Link Workers take a holistic approach to people’s health and wellbeing and connect them to community groups, statutory services, and activities to improve their wellbeing. Personalised care is integral, giving time and asking patients ‘what matters to me?’ giving more choice and control and reducing health inequalities. It is an exciting and varied role and is expanding into specialisms, including children’s social prescribing and substance misuse. Katrina was part of the team from Baywide PCN who was highly commended as part of the Self Care Forum’s Self Care Innovation Awards in 2021.
